What to Bring to the Oregon DMV

Bring everything in one trip. Most failed applications come down to missing documents, not failing the test. Below is the Oregon-specific checklist.

Identification documents

Acceptable identity documents

Documents must be original or certified copies from the issuing agency; photocopies are not accepted. Oregon DMV accepts one primary document or, if you cannot present one, at least two documents from its secondary list (see the DMV identity-documentation page for the full lists). The manual's most common forms of proof are: - U.S. government-issued birth certificate (hospital-issued certificates and baptismal records are not accepted) - U.S. passport or passport card, valid or expired no more than 5 years - Tribal ID card from a federally recognized tribe located in Oregon, approved by DMV - Certificate of Naturalization or Certificate of Citizenship - Employment Authorization Card (I-766), valid or expired no more than 5 years - Permanent Resident Card (I-551), valid or expired no more than 5 years - Foreign passport, valid or expired no more than 5 years - Re-entry Permit (I-327), valid or expired no more than 5 years - Refugee Travel Document (I-571), valid or expired no more than 5 years - Consulate ID Card from a country on DMV's approved list (e.g., Mexico, Guatemala, South Korea), valid or expired no more than 5 years - Oregon driver license, permit or ID card, valid or expired no more than 13 years - Out-of-state driver license, permit or ID card, valid or expired no more than 1 year If the applicant's current legal name differs from the name on the identity document, proof of legal name change is also required. Additional federal requirements apply for REAL ID cards (Oregon.gov/REALID).

Acceptable residency documents

Applicants must present one document proving current Oregon residence address. Common acceptable proof: - Certification by another resident at the same address (the certifier must accompany you and present their own license or ID) - Mail from a business, school, or government agency containing the applicant's first and last name (personal mail and forwarded mail are not accepted) - A document from an educational provider - A utility hookup order or bill, mortgage document, or property tax statement For a complete list of acceptable residence-proof documents, see OregonDMV.com.

SSN proof requirements

Applicants must provide a Social Security Number (SSN), or electronically certify under penalty of perjury that they do not have an SSN. The handbook does not require a physical SSN proof document (e.g. SSN card or W-2) at issuance for a standard Class C driving privilege; the certification step is built into the electronic application. Federally compliant REAL ID issuance has additional documentation requirements; see Oregon.gov/REALID.

Vision test required at issuance

DMV administers a vision screening at issuance for all original Class C driving privileges. There is no fee for the vision test. If the applicant fails, DMV provides a form to take to a licensed vision specialist. Wearing glasses or contacts during the test results in a "corrective lenses required" restriction on the license.
