Why the others miss

Entry and exit points do not have to coincide with numbered highway exits. This answer confuses HOV access points with the interstate's numbered exits, borrowing the mile-marker mental model.

B. At any point when the next lane is clear

A clear adjacent lane does not authorize crossing the striped buffer. This answer applies the 'change lanes when safe' rule from ordinary lanes and skips the location restriction the manual adds for HOV lanes.
